Change in practice attendance times for administrative queries

PLEASE NOTE A RECENT CHANGE IN PROCESS

To protect patients and staff, for any administrative queries please complete an eConsult or telephone the surgery after 10am. 

To register with North Street please use our postcode checker on our website to check you are within our practice boundary, once confirmed please download and email the completed forms along with your ID and proof of address to [email protected]. 

If you do not have access to the Internet or a clinician has asked that you collect any paperwork please attend the surgery between

  • North Street Site 12pm to 2pm or 5pm to 6.30pm
  • Ashton Gardens Site 12pm to 1pm or 5pm to 6.30pm
