How to Register
If you wish to register with the practice you must ideally complete the online registration form below or contact the surgery to register at reception.
To register please provide the practice with the following:
- Proof of Identity
Passport, Birth Certificate, Photo Driving Licence, Freedom Pass - Proof of Residence
Tenancy Agreement / Mortgage Agreement, Council Tax Bill, Utility Bill (no older than 3 months) Official Letter stating place of Residence (Home Office) - Patients from Overseas
Valid Visa (6 months – 2 years), Passports for all family members, Student Visa - Children Under 5
Birth Certificates or Passports, Child immunisation History, NHS Number - Proof of Residence

New Patient Interviews
New patient interviews are offered to all patients who are over 40 years old, if you are aged between 18-40 and you have a long-term condition, or on repeat medications.
All children under 18 years of age are required to have a new patient interview. If you do not booked or attend a new patient interview within 30 days after your registration has been confirmed, your registration will be cancelled.
If you are between 18 and 40 and feel you would like to have a new patient interview, please speak to a receptionist. All children under 18 years will be required to have a new patient interview. Please bring with you at the time of your appointment, a urine sample in any small clean container.
The practice operates over 3 sites North Street – Romford, Raphael’s House – Pettits Lane and Ashton Gardens(Chadwell Heath) Appointments will be offered at any site.

Duty of Care
North Street Medical Care takes its duty of care for our registered children and young people very seriously. Ensuring all children are up to date with their entitled vaccinations and immunisations is a priority for the practice. Medical evidence identifies significant increases in harm, clinical risk and poorer health outcomes for those who are not fully vaccinated and for children, this can lead to unnecessary vulnerabilities in their safeguarding and wellbeing.
If you are taking a number of medications you may be asked to make an appointment to see the doctor. If a New Patient interview is required, details will be taken of:
- any medication you are taking
- any chronic diseases you may be suffering from
- dates of immunisations etc.
